Prior to your visit, please read the following important information.
Plan Ahead
It’s important to plan ahead for your first visit to Urgent Care of Mountain View. At the time of registration, be prepared to provide:
- Prefilled Paperwork:
For a faster first visit, bring in your filled-out paperwork. Just download this forms, print it, fill in your information and give it to the receptionist upon arrival.
- Valid Photo ID:
You’ll need a valid photo ID, such as a driver’s license or passport, to verify your identity.
- Insurance Card:
If you have health insurance coverage, bring your insurance card with you. This will help facilitate billing and payment. We accept all major insurance plans, Medicare, Medicaid, and Tricare. We are currently in network with the following insurance companies:
- Aetna
- Blue Cross/Blue Shield
- Cigna
- Direct Net
- Medcost
- Medicare/Palmetto GBA, Medicare replacement plans, and supplement plans

- Credit or Debit Card:
If you are not using insurance or have a co-pay, be prepared to provide a credit or debit card for payment.
- List of Medications:
It’s important to bring a list of all medications you are currently taking. This includes prescription medications, over-the-counter drugs, and any supplements.
Important Info
Children under the age of 18 must be accompanied by a parent or legal guardian. Any other accompanying adult must have a notarized statement authorizing treatment, signed by the parent or guardian. All documents must be signed by a parent or legal guardian.
What to Expect
- Wait Time:
Your wait time will depend on the number of patients ahead of you and the urgency of their medical conditions. Our triage team will determine the order in which patients are seen.
- Check-In:
After arriving, you’ll check in with the receptionist. You’ll provide your ID, insurance card, prefilled paperwork and payment information if necessary.
- Medical History:
During your first visit, the medical staff will take your medical history to better understand your health background.
- Vital Signs:
Your vital signs will be checked by the medical team prior to your consultation.
- Consultation:
A healthcare provider will discuss the reason for your visit, listen to your concerns, perform an examination, then communicate their diagnosis, and provide a treatment plan based on your needs.
- Further Testing:
If additional tests or procedures are required, the provider will discuss your options with you.
Checkout
After your visit with our provider, you will be taken to the checkout area to receive a record of your visit and treatment discharge instructions.
Billing and Online Payment
Urgent Care of Mountain View provides patients with the convenient option to view and pay their bill online through our patient portal. You can easily login to your existing account, or create a new one using the information given to you during your appointment period. Its quick, simple, and helps you manage your payments effectively.
